Chapter 4 - The Poisoned Ledger

Harbor Ridge Medical Center rose like a sterile monolith against the stormy night sky. Rain poured down the concrete façade, streaming over the tinted windows of the private executive suites. It was nearly 2:30 in the morning, and the fourth-floor corridor of the cardiology department was deserted, bathed in the sickly hum of fluorescent lights.

Dr. Thomas Nolan sat at his expansive mahogany desk, reviewing patient files on his computer. He was a distinguished man in his late fifties, with silvering hair and gold-rimmed spectacles, projecting an aura of quiet authority. But beneath the facade, his hands were trembling as he poured two fingers of scotch from a hidden crystal decanter.

The heavy oak door to his office swung open and clicked shut.

Nolan didn't look up immediately. ""Nurse Jenkins, I told you I wasn't taking any more charts tonight—""

""I’m not a nurse, Doctor.""

Nolan froze. The crystal glass slipped from his fingers, shattering on the hardwood floor, spilling amber liquid across a Persian rug.

Julian Mercer stood just inside the room, tall, broad-shouldered, his dark coat damp with rain. He looked like an archangel of death that had stepped straight out of the storm. In his right hand, he carried a heavy steel medical case; in his left, the singed leather ledger.

""M-Mr. Mercer,"" Nolan stammered, scrambling to stand, his back hitting the bookshelf behind him. ""What... what are you doing here? If this is about Elena, I assure you, she is under the best possible care—""

Julian crossed the room with terrifying speed. Before Nolan could finish his sentence, Julian’s left hand shot out, wrapping like an iron vise around the doctor's throat. He lifted the grown man two inches off the floor, slamming him hard against the bookshelf. Books rained down around them—medical journals, textbooks on cardiac surgery.

""You have sixty seconds to give me the bypass frequency for the transmitter on my wife's chest,"" Julian said, his voice a low, terrifying growl. ""Every second you waste, I break a finger. You’re a surgeon, Nolan. You know exactly how difficult it is to set compound fractures in the hands.""

""I... I don't know what you're talking about!"" Nolan choked out, clawing desperately at Julian's wrist. Julian’s arm didn't budge an inch; it felt like solid stone.

Julian didn't hesitate. With a swift, sickening crunch, he bent the pinky finger of Nolan’s left hand backward until the joint snapped.

Nolan let out a muffled, agonizing shriek against Julian’s grip, his face turning purple, tears streaming down his cheeks.

""Fifty seconds,"" Julian said calmly, his eyes unblinking. ""Next is your index finger. You'll never hold a scalpel again. And after that, I will personally visit your son in Rhode Island and let Marcus Vance’s collectors tear him apart piece by piece.""

At the mention of his son, Nolan’s resistance completely disintegrated. ""Stop! God, please, stop!"" he wheezed as Julian loosened his grip slightly, allowing the doctor's feet to touch the floor. ""I’ll tell you! I’ll tell you everything!""

Nolan slid down against the shelf, clutching his broken hand to his chest, sobbing violently. ""I didn't have a choice! Vance came to me three months ago. He had video of my son... he had proof of the debts! He told me he would execute my boy and send him to me in a medical waste bag if I didn't cooperate!""

""The monitor,"" Julian demanded, leaning down, his face inches from the terrified physician. ""How does it disarm?""

""It requires a dual-key override,"" Nolan gasped, his breath coming in ragged bursts. ""There’s a radio-frequency transmitter in the monitor. It communicates with a base station that Vance carries. If the signal is severed, or if the heart rate exceeds one hundred and sixty beats per minute, the mercury switch triggers the detonation. But... but there is a physical key code. A four-digit sequence that must be entered on the device's side port while holding a specific RFID chip against the casing.""

""Where is the chip?""

Nolan reached with his trembling, uninjured hand into his desk drawer. He pulled out a small, metallic silver fob shaped like a coin. ""This is it. This is the master bypass for the surgical model they adapted. But you can't just punch the code in, Mercer! If Vance sees the signal drop from his base terminal, he can trigger the charge remotely via satellite!""

Julian snatched the silver RFID fob from the doctor's hand, inspecting it under the harsh fluorescent light. ""Where is Vance operating from?""

""I don't know! I swear to God I don't know!"" Nolan pleaded, cowering against the floor. ""He only communicates through burner phones. He told me to wait here until 4:00 AM. He said a courier would come to collect the medical records from Elena's monitor to prove she was still compliant.""

Julian stared down at the broken man. A year ago, he would have ended Nolan's life without a second thought. But tonight, looking at this desperate father who had compromised everything to save his child, Julian saw an ugly reflection of his own sins. How many fathers had Julian broken to protect his own empire?

""Give me the four-digit sequence,"" Julian said quietly.

""8-4-1-9,"" Nolan sobbed. ""8-4-1-9. I swear on my mother's grave, that's the code.""

Julian pocketed the fob. He reached into his coat, pulled out a thick roll of duct tape from the medical bag, and swiftly bound Nolan's arms, legs, and mouth to the heavy iron radiator beside the desk.

""If you lied to me,"" Julian said, leaning down to look the doctor dead in the eyes, ""I won't just come back for you. I will make sure Vance’s people find your son by sunrise.""

Julian turned and walked out of the office, his phone already in his hand. He dialed Mateo as he descended the fire escape into the dark, rainy night.

""Mateo,"" Julian said into the receiver. ""How is she?""

""Her heart rate is fluctuating, Julian,"" Mateo's voice was tense, filled with static. ""She knows you went out. She's sitting on the floor by the window, watching the street. The monitor just beeped three times. The rhythm is climbing. It's at one hundred and twenty.""

""I have the disarm key,"" Julian said, sprinting toward his armored SUV idling in the alley. ""Tell her I am five minutes away. Keep her calm, Mateo. Talk to her about Anthony. Talk to her about our first house in Cape Cod. Keep her heart beating slow.""

""Hurry, Julian,"" Mateo whispered. ""We’ve got a problem. Two black Suburbans just pulled up across from the penthouse lobby. They aren't Rourke's men. They’re carrying suppressed rifles.""
