Chapter 2 - Poison in the Inner Circle

The master bedroom in the Rainer estate smelled of antiseptic and lavender oil. Dominic hadn't left Elena's side for twelve hours. He had called Dr. Marcus Vance, a world-class neurosurgeon who had saved half a dozen of Dominic's soldiers over the years under strict non-disclosure terms, ordering him to the house under armed escort.
Elena lay propped up on plush pillows, the heavy white brace temporarily unbuckled under Vance’s careful supervision. For the first time, Dominic saw the full extent of the devastation beneath the plastic shell. Elena’s lower back was a topography of fading yellow-purple bruises, angry red abrasions from the brace's edge, and severe muscular atrophy. She looked so fragile that he felt his chest tighten to the point of suffocation.
""The L4 and L5 vertebrae are severely compressed, with a hairline fracture along the posterior arch,"" Dr. Vance explained quietly, holding up high-resolution scans on a portable light pad. ""Her local physician was trying a conservative stabilization approach, but frankly, Mrs. Rainer is one bad fall away from irreversible paraplegia. The fact that she walked on this for half a year is nothing short of sheer, agonizing stubbornness.""
""Can you fix it?"" Dominic asked, his voice low, dangerous with underlying dread.
""I can perform a minimally invasive decompression and lumbar fusion,"" Vance said, adjusting his glasses. ""Using titanium rods and synthetic bone matrix. But recovery will take months, absolute bed rest for six weeks, and intensive physical therapy. And Dominic... the surgery itself carries a five percent risk of permanent neurological deficit.""
""Do it,"" Dominic began, but Elena’s voice cut in, clear and resolute.
""No. Not yet.""
Both men turned to her. Elena pulled the sheet higher over her chest, her dark eyes flashing with that unyielding Lakewood baker stubbornness that Dominic had fallen in love with four years ago.
""I won't let you put me under general anesthesia while someone inside this house is trying to put you in a grave,"" she said, looking straight at Dominic. ""You need to hear what I saw that night.""
Dominic motioned for Dr. Vance to step out of the room. The moment the heavy oak door latched shut, Dominic sat on the edge of the mattress, taking Elena’s cold, delicate hands between his own. ""Tell me everything. Do not spare a single detail.""
Elena took a slow, measured breath, wincing as her ribs expanded against her aching torso. ""When that sedan surged over the curb, the streetlamp caught the driver through the windshield. He had a serpent tattoo wrapping around his neck, biting its own tail. Dominic, that is the mark of the Scarlatti crew from Chicago. The hitmen your family cut ties with three years ago.""
Dominic's brow furrowed. ""The Scarlattis haven't set foot in Ohio since my father died. They don't have the manpower to operate here without local sanction.""
""They had local sanction,"" Elena said, her voice dropping to a whisper. ""Three weeks ago, when I left MetroHealth, I saw the same black sedan parked three blocks away. A man got out to speak with the driver. I couldn't see the driver's face, but I saw the man handing him an envelope of cash. He had a gold signet ring with an onyx crest on his right pinky.""
Dominic's heart slammed against his ribs like a sledgehammer.
There was only one man in the entire Midwest who wore an onyx crest signet ring on his right pinky.
Luca Bellandi.
Luca, who had been Dominic’s father’s godson. Luca, who had sat at their wedding table. Luca, who had spent the last five months standing behind Dominic in the study, gently pouring bourbon and saying: She spends all day away from home, boss. A beautiful woman gets lonely when her husband is at war. Maybe it’s time to cut her loose before she breaks your heart completely.
The pieces of the puzzle slammed together with sickening clarity. Luca hadn't just tried to poison Dominic's marriage; he had orchestrated the hit outside the warehouse district restaurant. When Elena threw herself in front of the car, she hadn't just saved Dominic's life—she had inadvertently foiled Luca's first attempt to seize control of the Rainer empire. And when she survived, Luca had pivoted, using her secretive medical trips to fabricate an affair, pushing Dominic to divorce her and cast her out of the estate’s fortress-like protection.
Dominic stood up slowly. The air around him dropped ten degrees. The grief and remorse that had paralyzed him for the last day vanished, replaced by an ancient, predatory stillness that had made him the most feared underboss in the state.
""He tried to kill me,"" Dominic murmured, staring at the closed door. ""And when you took the hit for me, he tried to drive you away so you would die in an alley without my protection.""
""Dominic, listen to me,"" Elena said, reaching for his sleeve. ""Luca controls the northern docks now. He has half the lieutenants convinced you've lost your edge because you were obsessed with your failing marriage. If you move against him blindly, you'll start a civil war inside your own house.""

Dominic turned back to her. He leaned down, placing a gentle kiss on her feverish forehead, his hand cradling the back of her head with a tenderness he had withheld for far too long.
""Let him start a war,"" Dominic said softly against her hairline. ""He just made the fatal mistake of letting my wife live to tell me who pulled the trigger.""
