Chapter 5 - The Operating Table

Monday morning arrived with an ominous, gray sheet of rain pounding against the glass facade of St. Catherine Medical Center. Adrian’s security team had cordoned off the entire eighth floor, turning the surgical wing into an ironclad bunker.
Nora lay on the mobile gurney, dressed in a pale blue hospital gown, her hair tucked into a surgical cap. In the bay next to her, Maya was already hooked up to the pre-anesthesia IV lines.
“I love you, Nor,” Maya said, her voice slurred from the sedative. “Thank you for saving me.”
“I’d do it a thousand times, baby,” Nora choked out, reaching across the small gap between their stretchers to squeeze her sister’s fingers. “We’re going to be okay. Both of us.”
The double doors pushed open, and Adrian entered. Despite the sterile hospital environment, he wore his authority like an iron cloak. The nurses and junior doctors practically scrambled out of his path.
He stopped beside Nora’s gurney. For several long seconds, he just looked at her, memorizing the contours of her face. The cold, unfeeling mafia kingpin looked almost humanly terrified.
“Dr. Vance is the best in the state,” Adrian said, his voice surprisingly strained. “He has performed this exact procedure four hundred times without a single loss. You will close your eyes, you will sleep, and when you wake up, it will be over.”
“Adrian,” Nora reached up, her trembling hand catching his wrist. “If something goes wrong... if Maya has a complication... promise me Henry won't forget me. Promise me you’ll tell him I tried to come back.”
Adrian caught her hand, lifting it to his lips and pressing a fierce, lingering kiss against her knuckles. “Nothing will happen to you. I forbid it. Do you understand me, Nora? You are coming back to us. To Henry. To me.”
Before she could process the weight of his words, the anesthesiologist wheeled the cart forward, injecting the clear sedative into her IV port. The sterile room began to tilt. Adrian’s dark eyes were the last thing she saw before darkness dragged her under.
Two hours into the complex dual-transplant surgery, the lights on the eighth floor flickered violently.
Adrian, pacing like a caged beast in the private waiting suite, stopped dead in his tracks.
Pop. Pop.
Down the hall, the secondary emergency lights buzzed to life, casting an eerie crimson glow across the corridor. Simultaneously, the fire alarms remained dead silent—a calculated sign of deliberate sabotage.
Marcus burst into the suite, his weapon already drawn and held low against his leg. “Boss! The primary power grid just got cut from the basement junction box. The backup generators kicked in, but the surgical telemetry monitors in OR-3 just went black!”
Adrian’s eyes narrowed into terrifying slits of pure predatory fury. “Where is Julian?”
“He left the building twenty minutes ago claiming he had to oversee a wire transfer at the downtown office.”
Adrian didn't need any more evidence. The betrayal was immediate, precise, and intended to execute Nora while she was paralyzed on an operating table.
“Lock down every exit,” Adrian snarled, pulling a customized matte-black pistol from beneath his jacket. “Shoot anyone who tries to leave the basement. Marcus, take four men and secure OR-3. If anyone other than Vance’s verified surgical team steps within ten feet of Nora or her sister, put a bullet in their forehead.”
Adrian marched down the hallway, fury radiating off him in suffocating waves. He reached the heavy scrub doors of Operating Room 3 just as a man dressed in surgical scrubs was attempting to slip out the side utility stairwell, carrying a discarded medical cooler.
Adrian didn't ask questions. He surged forward like a phantom, slamming the man against the concrete stairwell wall with enough force to crack plaster. His hand clamped around the man’s throat, lifting him until his feet dangled off the ground.
“Who sent you?” Adrian whispered, the muzzle of his gun pressing beneath the man’s chin.
The impostor choked, eyes bulging in horror as he recognized the Devil of Philadelphia. “L-Luciano... Dominic Luciano... he said... he said just switch the saline bag with potassium chloride... please!”
May you like
Adrian didn't waste another breath. He threw the man down the stairs to his waiting guards with a single command: “Keep him alive until he sings on camera. Then dispose of him.”
Turning back toward the operating room, Adrian stared through the circular observation window. Inside, Dr. Vance was manually ventilating Nora while the auxiliary monitors beeped back to life under emergency generator power. Nora’s chest rose and fell rhythmically, unaware of the blood that had almost been spilled in her name.
